Registration

You're walking through the corridors of a large school, seemingly vacant of people from your old school. You were convinced to transfer to another school one month ago; the problem is, there is no information whatsoever about this school. You've spent most of your time trying to find even one sentence about Olympus High, but there was nothing.
The sound of shouting alarms you, coming from outside the school. They are shouts of battle, one that gives you a rushing feeling you haven't been able to describe yet.
You decide to ignore it (however much you could, anyway) and made your way to the front desk, where a very cordial looking man, dressed in a suit. He looks up at you, smiling for a brief second before slipping a paper from a somewhat large stack and handing it to you.
"What is this?" You blink at the words, surprised that you understood it perfectly, being dyslexic and all.
"You wanted to register, correct?" The man never strayed his attention from the computer sitting on his desk, clicking a file quickly.
"Oh, um, yeah." You take the slip from his hand, reading it through. Its contents were:
